By genre the game is a 4X survival base builder. The player is a plague doctor who, after an epidemic, restores the Sanctuary, the main building of the city, and builds everything else around it. Progress runs in two directions at once. The city grows through buildings and research, while the heroes and their squads defend the territory and capture someone else's.
